You should read the financial data portrayed in the reprints. The transfer of technology white paper displays the research which supports the existence of a parallel universe away from the academic medical center. Further, it failed to tabulate ancillary revenues such as ultrasound and related surgical services.
The key to understanding this important idea is the historical fact that large corporate organizations such as medical schools resemble General Motors and IBM in the 1970's. They ignored the emergence of Toyota and Apple computers. General Motors has been losing market share every year for over 20 years and IBM has abandoned the personal computer market. They were too late to compete.
It would be worth a noon conference dedicated to reviewing these data for students with an interest in health care economics. Millard, could we do a conference on this for the residents. Your comments are appreciated.
