No, but we seek the support of the board to recognize the need for a global presence. The opportunity is large. Dan, would you be interested in working with me to study this idea? In addition to the course below, can we work together on an ALSO course this summer? This is a prototype on a small scale. Please do not post to other national listserv's.
Members of the Medicos family and other colleagues are involved with Africa, Central America, the Mideast, and South America. We have some people on the ground in Venezuela, Ecuador, Guatemala , and Argentine with the hope of establishing such programs. The African programs are already up and running but they add basic general surgery/ortho. I hope we can recognize them as qualified.[see Pust, Dahlman Fam Med Oct 2006]
Although Medicos' basic curriculum has not changed, the Medicos program is relabeling its concept as "Surgical Family Medicine Obstetrics". This relabeling is partly in response to the need for an international identity. Also this was the major concern of the dialogue group at the Society of Teachers of Family Medicine 2009 and 2010.
Donors have endowed a lectureship dedicated to family medicine obstetrics. In collaboration with UT surgery, who will be providing use of their animal laboratory, Medicos plans for the a continuation of our Surgical Family Medicine Obstetrics Conference Labor Day(pun intended) in Memphis Sept 4-6. This is a limited registration course at no cost for faculty. This conference will highlight higher risk OB management through higher level ultrasound and skills in surgical obstetrics. Both of these skills are not available in most family medicine residencies. [Fam med 2010; 42: 712-716]
I am asking the Board of Family Medicine OB to be a sponsor(at no cost) with Medicos. We agreed in principle to creating an ABPS BFMOB CME event.Here is a preliminary outline of that CME. All teaching will be at the Medicos conference facility which contains 5 ultrasounds, 2 OR's, 15 exam rooms, skype, etc. It simulates a small mission hospital.
The focus of this conference will be Quality Improvement of Pregnancy Care--Advanced Diagnostic Ultrasound Workshops and Surgical Skills Laboratory.
Participants Will:
1. Receive a handout describing goals and objectives. Each of five common ultrasound examinations will be described on a one page medical record report form.These forms constitute the recommended minimum data for clinical situations most frequently encountered in rural communities and developing countries. These will be preprinted in English and Spanish.
2. Receive an overview of requirements for a Certificate of Achievement in Diagnostic Ultrasound. English and Spanish.
3. View and respond to a programmed videodisc describing competency expectations for obstetrical ultrasound basic and advanced. Video clips will be used to simulate real time examinations.
4. View and respond to a programmed videodisc simulating real time examinations for common emergencies.
5. Use pelvic simulators to perform transvaginal examinations detecting ectopic pregnancy and free fluid in the pouch of Douglas.
6. Conduct live ultrasound examinations on patients. This includes tranvaginal examination.
7. Tests will be demonstrate for the purpose of establishing curriculum validity. This is a beta test.
a. Oral exam with live patients for skill in the transabdominal quick look exam
b. Oral exam with live patients for assessing skill in the transabdominal 2nd and 3rd timester examination.
c. Oral exam with live patients for assessing skill in the transvaginal examination. Physicians will be required to correctly locate and measure the uterus and ovaries
d. Written test with multiple choice questions--Board simulation
8. Surgical equipment overview
a.Role of office surgery for the preliminary assessment of skill in choosing needed equipment, needle size, anesthesia, suture size, wound care.
b. Clinical simulations and case reviews of common office surgeries.
c. Hernia Repair on the mission field--brief overview of surgery under local
9. Tissue workshop in vivo
a. Laboratory etiquette, anatomy variations, and assignment of teams
b. Incision basics and needle etiquette
c. Repair of inadvertent enterotomy
d. Repair of inadvertent cystotomy
e. Repair of intestinal portion of fourth degree laceration
